Server form not working

So I tried submitting a form to get a whitelist on the MCM server and well this happened:

(When copying the full content of the site [id & name])

(When only putting in the uuid)
http://prnt.sc/bmqwuj

And the same thing happens when I put in the dashed UUID. So yea, anyone else having this problem?

Edit: Added Minecraft association and successfully submitted the application form. Requesting for lock.

I just went on namemc and copy and pasted only my UUID, so instead of the id thing but just the numbers and letters.
