Animated Server Banner

I'm looking for an animated banner for my server.
I would like it to display my server's logo, along with animated/moving text with info and small effects.

I don't have a set budget for this, so please post your prices below and a link to your portfolio.
Also, I would like the project files to be included so I can make changes myself if needed. If this isn't possible, then I would like to know the cost for making changes.
