BungeeCord Report Plugin

I'm looking for a custom plugin for BungeeCord, it's a Report plugin.
Features I need:

- /report <player> reason ( no permission )
- Staff receive ( report.staff )
- Report cooldown time.
- You can not report yourself.
- Customizable messages.
- Teleport to the server when you click the report.

Report Format:
=======[REPORT]========
Player Reported: <player>
Reported by: <player>
Reason: <reason>
Server: <server>
=====================

Doesn't a plugin such as this already exist?

I would check out Bungee Report: https://www.spigotmc.org/resources/bungee-report-1-8-1-9-network.7564/

All messages are configurable, and you should be able to get it how you want if you know what you're doing!

With that plugin you can report yourself, also you can't teleport to the server.
