It obviously has to agree with there privacy settings though because you can't see some user's birthday.
Before their name or on their profile picture would be cool.

This could be a really funny suggestion

But probably add an option for those who dont want that cake on their birthday.

It's really not worth custom development for something like this. If you've found an existing addon for something like this please let me know in a new suggestion thread, but with the amount of stuff we have to make it's not reasonable for us to do.
