Contest/Giveaway Form - Organization and Prevention of Fakes

Recently I've noticed that many giveaways have no end date, require social media interaction, get abandoned or otherwise break the rules.

I think it would be beneficial to everyone if we had a required form as seen in the image below.

This would ensure that all Contest/Giveaway rules are being followed and will help prevent fake contests/giveaways.

Yes, because of that I saw how many threads get abandoned by their creators or otherwise broke rules.

How much inspect element did producing that picture take?

I just changed the text on the scam report form and removed extra boxes.

To the looks of it, all it looks like it is doing is making sure people provide enough information.

Its not really forcing people to make sure they give something away. (most people forget about their giveaways)

at least they could set an automatic end date after which it is locked, or after which staff get a notification to check if it it has been ended yet. warnings can be given.
It should have a maximum duration though.
