Custom item textures (PLEASE HELP!)

Currently I've been attempting to create custom armor and swords for my server. A great example of what Im trying to achieve is Hypixel's Warlord Minigame.

I've attempted numerous ways of trying to get custom textures but I am completely clueless now. I have hit a dead end and I am really hoping a developer is willing to help me out.

I would greatly appreciate it and I would go out of my way to attempt to benefit you as you benefited me!​
