Discord/Minecraft Account Linking Plugin

Hey! I'm looking for a plugin / bot that can link a players in game account with their minecraft account. This would be for sponge/spongeforge 1.12.2 (my server is a pixelmon server)

The bot would change the players nickname to their In Game Name

In game, the player would receive a series of rewards run by console
