I can already tell you your problem is you're using Group Manager

Groupmanger is stable... has more config imo

Groupmanager hasn't been supported since Essentials dropped development in 1.7.4. It is the least stable Permissions plugin. There's nothing to config in a permissions plugin except ladders, which GroupManager doesn't even have.
