Help with PHP and SQL

Hey.

I'm doing a project and I have all front end work done, Here's what it is:

We have a form which takes in:
"FIRST NAME"
"LAST NAME"
"PHONE NUMBER"
"ADDRESS"

and whenever a log with all those entries is complete, it logs on a table in a file called "database.html", also, whenever a log with all those entries is complete, It needs to have a seperate ID number for every log in the table so it's never the same.

Before you wonder what this is, It's for a marketing company and it is 100% legal.

I just need some help completing it, I can offer payment.

Tell the marketing company to use slack and you can easily manage the business from there and also setup fields of storage.
But to fix what you are doing, I'd need more info on how you set this form up.
