Make sure your webhost has PHP and MySQL.
Unzip the Poll.zip and move the contents of the 'poll' folder into the directory on the FTP server where you want the site to load.
Edit the config.php file accordingly (hostname, port, username, password, database) and save it to the FTP.

Seems like your host doesn't have the MySQLi extension.
You can ask them to enable it, or get the developer to rewrite it using something else (PDO? Depends what your host has enabled).
