I Need Help Fixing a Glitch With /v

Hello,
I am currently a staff member on intensity craft, a minecraft factions server. Lets say Player A and Player B are both staff member capable of doing /v. Player A is on the server not in vanish. When Player B joins and it looks like Player A has /v, but he/she isn't. Player A has to type /v two times in order for Player B to see Player A. How do i fix this?
