I was using a cutoff wheel at work used to cut through metal, and the disk was broke and got me right in the jaw. This is a cutoff wheel if anyone doesn't know

Seems to me like the equipment wasn't maintained properly

I literally put on a brand new wheel on my tool, apparently it dropped or fell off the ledge connected to the air line that I had it on, walked around the conveyor, and didn't see the wheel busted, I hit the trigger and it nailed me right in the face
