Litebans web interface

Hey all, i have the litebans plugin and i have a website etc, but i need help setting up the bans web interface, if you can help me please message me on mcm

I have done the MySQL stuff, but it's giving me this error: http://prntscr.com/eucbf4 and in-game it says that it can't connect to a database... here's my code:

Edit:

1. "address: localhost:3306" is now my *mywebsiteaddress*:3306.
