Logo and Banner Design for YouTube

I am requesting a quote for a Logo and Banner design for a YouTube channel. I do not currently have a exact idea what I want but I have a picture and style for the logo. The banner does not need to be anything crazy, just a simple text banner. I want it kind of in the basic style of this logo, with some sort of item in the middle. It would have C on it.
