Minecraft client issue

for some reason when i login on a server i get this issue^^^

will not let me click respawn or title screen.

I made an item before that did this glitch, you could only exit it using /kill from another person or console. I gave the item meta tags with -10000000 damage or something, if you held it for too long it kills. Also that was in vanilla.
