Nether-cave like terrain 2-layered map

So basically ive talked to a couple of Terraformers and they are unsure of how to create a nether/hollow/caves type of terrian within a map unless done manually done by world edit. Anyone able to do something like this? What im looking to do is have a map made, the surface is super barren right, no vegitation, lifeless, but when you go underground you find that there is this kinda cave world/underworld that is vegetated, colorful, and has life.
