So in the server staff section there a load of "requests" for staff, then the OP says something along the lines of: "Fill this out:
Name:
Skype:
Etc..."

Then people don't follow the format the OP wants at that takes up space etc...

I think there should be an option like the "add a poll" option put instead you can make a form, like the ones in Scam Reports and Contests and add optional options like: If 'age' < '18' don't post or "automatically put post in spoiler".
