Server Portal Page

Hello, I am interested in a server portal page such as http://www.datpixel.net/ http://primemc.org/ and http://nerd.nu/

All of those portal pages have what I want, I just want something basic (Probably not as advanced as PrimeMC's lol) that has a nice background, I'm willing to supply a background image, links to the forums, buycraft, homepage, and voting links, and lastly the server IP.

If anyone can give me an idea of what getting one of these made for me would be like and the associated price along with that, please do so!
