Thread templates

What I'm requesting here is somewhat different of what you might be used to.

At the moment, I'm running a moderately successful premade builds shop. However, the thread is a mess, and I'm not good enough with thread layout design to fix it.

I'm NOT asking for a standard thread design!

So I'm looking for the following:
A template, made in Photoshop, Illustrator or GIMP, for 5 things;

• A title. This will be where I state what type of product I'm selling (PvP arenas, for instance)
• a sub-title. This is where I'll be naming the specific product
• An image post: This is where I'll be posting at least 1 image of the actual product
• a description post: where I will describe the product, if relevant. Fit for large blocks of text
  
• a Price/buy now "Button". This allows me post a price and turn it into an image link

I want all of these to be separate images following the same theme. By "Template" I mean that I can re-use them by changing the content, while still following the style of the design.

I will give you permission to use any and all of the renders of my Portfolio for this project, since I have made those myself. You can also use them in the digital sketch:

Since this is my first time buying graphics, I have no set budget. I'll be looking for as many different offers as I can, however, so keep that in mind when making me one.

Ideally, I'd like an offer to consist of

• a rough sketch (digital or physical I don't care, as long as it's legible) of how you'd do each. Does not need to be detailed, as long as you can use it to explain your idea.
• An initial color palette
• A price (or price-range)
  
• a completion time
